AIM HIGH Adult Parent Group Meeting: Information Forum on Care Coordination Organizations
The next meeting of the AIM HIGH Adult Parent Group will be held on Thursday, January 25, 2018
DID YOU KNOW OPWDD (the Office for People with Developmental Disabilities) has said that current Medicaid Service Coordination services will transition to health home care management services under Care Coordination Organizations (CCO) on July 1, 2018. NYS Health Homes assist individuals on Medicaid by providing personalized care management services so people can more efficiently and effectively access healthcare, social services, and community and natural supports. A CCO will be a specialized Health Home for individuals with intellectual and developmental disabilities (I/DD).
This forum will provide an opportunity to learn more about the transition from Medicaid Service Coordination to Care Management under a Care Coordination Organization.
